Making a Referral

Referrals should be made directly to Sue Rodriguez John and may come from health and social care professionals, other service providers and directly from potential service users and their families, carers and supporters.

Initial enquiries can be made to Sue by telephone on 07557880648 or e-mail at suerodriguezjohn@vistacareltd.co.uk, but a written referral should follow before any decision is made to consider a potential service user.

Once an enquiry is received, potential service users, family members and professionals are invited to visit our homes. This will give them the opportunity to meet the Home Manager, staff team and to learn about service delivery and the opportunities which are available to them. Detailed information will also be provided which includes; statement of purpose, sample care plan, sample menus and sample activities planner.

If the decision is made to proceed then a full care needs assessment is carried out and a transition plan and care costings package is developed.
